Intake Appointment: $120 for the 50-minue intake appointment and follow up paperwork
Individual Therapy: $120 per 50-minute session
Family Therapy: $120 per 50 minute session for up to two people present
If you are interested in receiving services and requesting possible reimbursement from your insurance provider, I am able to provide you with a monthly statement or "Super Bill." If your insurance covers "out-of-network providers" (typically PPO insurance plans) they may reimburse you for part of my fee; however, it is important that you contact your insurance company prior to services in order to determine what the reimbursement rates are on your plan and what information is required for reimbursement.
IMPORTANT NOTE ABOUT USE OF INSURANCE:
It is important to keep in mind that health insurance is diagnosis-driven, meaning that your insurance will cover therapy only if there is a certain diagnosis that the client can be given. If the client does not have a diagnosis or if I assess that the client does not meet the criteria for a diagnosis, insurance cannot be billed. However, clients using a cafeteria plan of pre-tax medical spending can request an invoice from me and this invoice may allow the client to use the established tax free benefit without needing a diagnosis (please check with your provider in advance to clarify your benefits).
Reduced fee services are available on a limited basis.
Payment is due at the time services are provided and cash or check payments are accepted. Checks for therapy services are made out to: Shelley Balla-Hawkins.
Your appointment time is established and held especially for you. If you do not show up for your scheduled therapy appointment, and you have not provided at least 48 hours advance notice, you will be required to pay the full cost of the session.
